New Patients

New Patients:

We are thrilled to welcome you to Anchor Dental, where your dental health is our top priority. Thank you for choosing us to be a part of your oral care journey. Our dedicated team is committed to providing you with exceptional dental experiences in a warm and friendly environment.

Getting Started:

To streamline your first visit, we invite you to fill out our New Patient Documents. You can either print them out and bring them completed to your first visit OR you can fill them out online and email them to us.

Submitting X-Rays:

If you have any recent dental x-rays at another office, we encourage you to email them to our office prior to your first visit. This will allow our experienced dental professionals to review your records to ensure a comprehensive and efficient examination during your visit. If you are unable to obtain these records before your visit, new dental x-rays may need to be taken to complete your exam.

Your First Visit:

Upon arrival, our friendly staff will be here to greet you and guide you through the check-in process. Please bring your new patient paperwork, photo ID and any dental insurance cards. We want you to feel comfortable and informed every step of the way. During your first appointment, one of our doctors and team will take the time to get to know you, discuss your dental history, and address any questions or concerns you may have.

Insurance:

Our office participates in some insurance companies. If you would like to ensure that we are in network with your insurance, please contact us directly. Rest assured, even if your insurance is out of network with our office, we will be happy to submit claims to any PPO plan on your behalf.

At Anchor Dental, we believe in building lasting relationships with our patients. We look forward to being your trusted partners in achieving and maintaining optimal oral health.

We highly advise that you submit the completed forms online to expedite your check-in process when you arrive for your appointment.
